Important information

Minor children who wish to apply for a travel document require the permission of the person(s) in charge of custody. 

Consent is required in the following situations:

  1. A child under the age of 12 when applying for an identity card;
  2. A child under 18 when applying for a passport;
  3. An adult who is under guardianship

Who should sign? Claimant(s):

a. Both parents if they have joint custody (Note! Also if the parents are divorced unless otherwise determined by the court);

b. Mother, when she has sole parental authority;

c. Father, when he alone is entrusted with parental authority;

d. The body exercising authority;

Notice! This form must be signed by all persons with custody.
