To complete the application, the operating permit holder should complete form WS-2123-1 'Declaration of possession of operating permit gaming machine manufacturer/landlord with DigiD' . Through this form the operator (lessor of the machines) declares, that he/she is in possession of an operating license. This concerns the license issued by the Government of Rijskoverheid.
Do not make the application, WS-2122 "Application for Attendance Permit for Games of Chance Machines," until the above has been completed or is about to be completed. If it is not completed, your application is not complete and cannot be processed.
