To complete the application, the operating license holder must fill out FormWS-2123-1, “Declaration of Possession of an Operating License for a Gaming Machine Manufacturer/Lessor with DigiD.”Through this form, the operator (lessor of the machines) declares that he or she holds an operating license. This refers to the license issued by the national government.
Do not make the application, WS-2122 "Application for Attendance Permit for Games of Chance Machines," until the above has been completed or is about to be completed. If it is not completed, your application is not complete and cannot be processed.
