To complete the application, the operating license holder should complete form PU-2123-1 'Declaration of possession of operating license for gaming machines manufacturer/landlord with DigiD'. Through this form the operator (lessor of the machines) declares, that he/she is in possession of an operating license. This concerns the license issued by the Government of Rijskoverheid.
Do not make the application, PU-2122 'Application for presence permit for gaming machines', until the above has been completed or is about to take place. If it is not completed, your application is not complete and cannot be processed.
