WS-2123-1 Declaration of possession of operating license of gaming machine manufacturer or landlord with DigiD

Procedure:

1. You log in with DigiD.
2. After logging in, you will come to a form, part of which has already been filled out for you. Please check this information and carefully fill in the rest of the form.
3. You will then be asked to add the listed attachments.
4. After completing the form, you will immediately receive an acknowledgement of receipt.
5. If there are ambiguities or questions, you will be contacted.
