Did you incur costs to unclog your sewer and it was determined that blockage was in the municipal section please fill out this form.

The municipality assesses your damage. If the municipality finds that it is liable, you will be reimbursed for all or part of the damages.

Good to know:

For better and quicker assessment, the congregation requests at least:

• A description of the event (place, date and time).
• Any photos of the event. These can be attached as attachments. Please have these ready digitally.
• Invoice of expenses incurred from contractor. Keep these ready digitally.
• Contact details of the specialist(s), in case you have engaged them.
