WS-2134-2 Change supervisor for Alcohol license with eRecognition.

Do you already have an Alcohol License and there are changes regarding the manager(s)? If so, please report these changes.

Good to know:

• To add an executive, you do not need to reapply for a license.
• This form allows you to add one or more executives.
• This form also allows you to remove one or more executives.

For better and faster treatment, the congregation requests at least:

• Licensee data.
• Details of the last Alcohol License issued
• Whether there has previously been an amendment appendix associated with the Alcohol License. If yes, when?
• Details of the new supervisor and what certificates/diplomas the supervisor has.
• The supervisor's start date and whether he/she is salaried.
