Filing a Wmo notification

Actively participating in society and continuing to live independently is important to many people. The Social Support Act (Wmo) regulates that the municipality can help you if you cannot do everything yourself. For example, because of your age, disability or illness.

Do you need support or assistance? Then report this to the municipality using the Wmo notification form:

Explanation of DigiD

You can fill out the form with a DigiD. This is your digital proof of identity.

With your DigiD you show who you are when you arrange something on the Internet. You can log in securely with it. Your personal information remains well protected.

Don't have a DigiD? Or are you reporting on behalf of someone else? Then choose the form without DigiD.

In it, you must enter these details about the person who needs help:

  • first name
  • last name
  • date of birth
  • zip code
  • house number
  • what help is needed

Can't the municipality consult with the person who needs help? Or can't that person make decisions for themselves? Then enter:

  • Who the congregation can contact (a contact person)
  • Who gets to make the decisions (a proxy)

How does it work when you file a Wmo notification?

If you want support from the municipality, apply for it in these steps:

Step 1: make a notification

You file a Wmo notification using the form at the top of this page. Have you completed the form? Then you will receive a confirmation by e-mail.

We will call you as soon as possible to discuss your report. We may need more information from you. Or we may have advice for you. Sometimes, for example, there are organizations that can help you and support from the Wmo is not needed.

Is the conclusion of this telephone conversation that we are going to investigate whether you can get support from the Wmo? Then we will schedule an interview at your home (see step 2).

Step 2: interview at your home

A Wmo advisor from the municipality will visit you for an interview to explore what is possible. We call this the kitchen table conversation.

The conversation will include your personal situation. Feel free to ask a family member, friend or acquaintance to join this conversation. You can also ask for help from a client-support person. This is someone who can support you free of charge.

Together with the Wmo counselor, you may still come up with solutions. Such as:

  • buy, rent or arrange something yourself that makes your life easier
  • help from a welfare or volunteer organization

Does this not help enough? Then the Wmo advisor will investigate whether you can get help from the Wmo.

When is the interview? 

We try to schedule the interview within 6 weeks of your Wmo notification. We are sorry for you that it is taking so long. Unfortunately it is very busy. We do our best to keep the waiting time as short as possible.

Step 3: conclusion of the study

You will receive a report of the conversation and a conclusion after several weeks. This will state:

  • what you can do yourself
  • what you need support with
  • Whether you can get help from the Wmo and if so what kind of help
  • what arrangements you have made together

Are there more solutions for you that are about as good? Then the municipality will choose the cheapest one.

Step 4: applications

After you receive the conclusion, you can apply or arrange for help. It goes like this:

Help from Wmo: Does the report state that you can get help from the Wmo? And do you agree with the help you will receive? Then sign the report and send it back to us. This is your official application. If your application has been properly received, you will receive a letter from the municipality stating that you will receive the support. This letter is called the decision.

Help via another route: You may have heard that you will not receive help from the Wmo. For example, because you can get better help somewhere else. Then we can help you how to arrange this help. We always try to find a solution together.

Do you disagree with the report? Then you can let us know. This is called objecting.

How long do you have to wait for support?

Companies are required to provide the help or aids within 8 weeks after you are approved for the support. The company arranging your aid or resource will contact you.

Do you want to know approximately how long it will take? Then contact the company yourself. You will find the e-mail address and phone number in the approval letter (the decision).

Temporary solution if you have to wait

It is not pleasant for you if you have to wait for support. We are happy to think with you about a temporary solution to your problem. Please contact the Social Team.
