Why did I receive a letter
You are receiving this letter because we are concerned about your personal situation. You have recently been in contact with the police and/or justice and/or social services. We would like to help you. To be able to help you, different organizations will talk about you together and make a plan. They do this within the Local Chamber in Wassenaar. After this you will get a contact person and this person will tell you what has been discussed.
What does the letter say?
The letter says that several organizations want to talk about your situation because they are concerned about you, and one of these organizations has reported your situation to the Local Chamber. To make a plan to help you, a few things are going to happen.
Personal data capture
We include your information in the Local Chamber's PGAX registration system. The information may only be used of the as agreed in the privacy covenant Local Chamber Wassenaar. In short, this means that your personal data is only processed if it is necessary to help you and it falls within the framework of the partners' job descriptions.
The following data about you will be recorded:
- Name, address and date of birth
- Information about your situation and the caregivers or agencies involved;
- The plan of action to help you.
Your rights
The local chamber considers it very important that your privacy remains guaranteed. Therefore, a privacy protocol has been established that clearly explains how your personal information is handled in the local chamber.
You also have a number of rights about your data processed at the Local Chamber. What these rights are and how you can exercise them can be found in the Local Chamber's privacy notice.
